Prior to attendance, in accordance with the State of Montana requirements, Carroll College requires the following;
- Proof of a meningitis vaccination within the last 5 years (NOT meningitis B)
- Proof of a PPD (tuberculosis) skin test within one year prior to attendance
- Proof of a tetanus shot within the last 10 years and series
- Proof of 2 MMRs
- Proof of polio series
Completed immunization forms are due before orientation.
Applicants to Carroll College must submit a letter of recommendation as part of their application for admission. The recommendation may be a letter or this form. Transfer students are encrouaged to submit a letter rather than this form.
